Health Care ServicesSummit View of Lake City offers a complete range of Health Care Services to meet each resident's needs. Case Management. At Summit View the individual resident's status and needs are continuously assessed by a skilled team of experts. The team includes the medical director, nursing and therapy staffs, and other specialists as needed (such as nutritionists, podiatrists, etc.), as well as the resident and family members, who are closely involved in this process. Case Management at Summit View means that the resident's needs always come first. Physical, Occupational and Speech Therapies. The objective is to return residents to their highest level of functioning so they can return to work, home and life as before. Through ongoing Case Management, our staff assesses needs and sets goals, which are discussed with the patient and family, and implements needed care and follow-up.
Occupational Therapy. Focuses on activities for daily living, such as dressing, bathing, transfers, toileting, cooking and other routine tasks to promote independence prior to discharge. For those who are close to being discharged, our "Almost Home" program provides a simulated home environment in which to practice the skills needed for independent living. The goal is to identify possible problems early, thereby increasing both safety and the likelihood of success. Speech Therapy. Evaluates and treats disorders relating to cognition, communicating and swallowing, which can result from surgery, traumatic brain injury and stroke. This therapy increases the patient's safety and maximizes the ability to communicate wants and needs. Enteral and Perenteral Nutrition. A resident having difficulty swallowing or other problems can still receive the nutrition they need. Summit View can provide this vital therapy either enterally (into the digestive system) or perenterally (into the circulatory system), depending on individual needs. Greenbrier Special Care Unit. Dedicated to the treatment of Alzheimier’s disease and related disorders, this unit is staffed by professionals who have been specially trained to care for those suffering from dementias. We offer specialized activities that stimulate minds that may seem to be closed. Greenbrier is a secure environment of openness and freedom of movement, without sometimes-confusing hallways.
